At Little Whispers, we take a compassionate, collaborative, and strengths-based approach to care. We believe that every individual and family deserve personalized support that reflects their story, values, and goals.

Assessment Process

Each journey begins with a comprehensive, strengths-based assessment. Through respectful
conversations and professional tools, we learn about each client’s needs, preferences, and
priorities. This helps us design care that feels personal, meaningful, and effective.

Individualized Care Plans

Following assessment, we create an Individualized Care Plan (ICP) tailored to the person’s unique circumstances. Clients and families are active participants in this process—they help shape goals, strategies, and timelines to ensure the plan truly fits their needs. Every plan is reviewed regularly to remain relevant as clients grow and circumstances evolve.

Coordination of Services

We collaborate with other professionals, community resources, and support networks to provide seamless, holistic care. With consent, our team coordinates communication and planning to avoid overlap and ensure everyone involved is working toward shared outcomes.

Communication with Families

Families are essential partners in the care process. We keep communication open, ongoing, and transparent through regular updates, discussions, and check-ins. Confidentiality and privacy are always upheld with the utmost respect.
